HTA e-form software

This file is a Windows installer package which is used to install the Toplevel OfficeForms filler software on your computer. You need this software installed to use HTA electronic proposal forms. Instructions for installing this software are given in the above guide. Download e-form software (msi file, 4mb)

HTA Clinical Trials e-proposal form

The electronic proposal form is used with the HTA e-form software programme and is currently being revised. It will be available on this page from the end of May 2008. Please note that applications received on previous versions of the proposal form will not be accepted.

Important: the electronic proposal form will NOT work on Apple computers including 'Macs'
